Our goal is to work closely with your doctor to help manage CHF symptoms to reduce your chances of hospitalization and keep you in the comfort of your own home. We also focus on patient education, self-management, medication management as well as compliance to maximize patient outcomes.

Our home health care team is focused on working with your doctor and together developing a COPD Management Plan in order for you to manage your COPD symptoms through self-care while being able to remain at home living as independently as possible. Our home health care team will help provide education, tools, and support so you can achieve your personal goals and improve your overall health.

Golden Care’s home health Diabetes Program is built to empower patients by educating them on compliance, symptom management, and with the help of your doctor – an effective treatment plan to maintain a health lifestyle. Our program engages the patient in self-care and educates on the importance of monitoring blood sugar levels and recognizing signs and symptoms of diabetes related complications. These interventions in return will help the patient to achieve goals and improve overall quality of life.
